The patient is a sixty-three year man with chronic hypertension, left ventricular hypertrophy, and diastolic dysfunction who had undergone bilateral renal artery angioplasty at an outside institution in February 1998. This procedure was unfortunately complicated by complete renal failure necessitating hemodialysis over the ensuing nine months. However, the patient continued to make urine throughout this period and his serum creatinine stabilized at 3.2% in January 1999 at which time he was released from hemodialysis. Renal ultrasound in September 1999 revealed right and left kidney sizes of 11 and 7.7 cm, respectively. An MRA scan in October 1999 revealed bilateral renal artery restenosis (above).
